Joan Kirner (JK) = women’s and children’s (for ortho this means paeds <18yrs only)
Joan Kirner used for clinics (small radiology attached), children’s admissions and theatre
Paediatrics will be seen in Sunshine ED and transferred over to JK for admission)
Sunshine used for adult admissions, theatre, ED and ICU
Sunshine = bigger and busier hospital than Footscray with more ED presentations
Referrals often come later in day as patients wait longer in ED and lots of medical teams rounding throughout day then referring in PM
Generally all NOFs and obvious adult trauma BIBA is taken straight to Footscray but self-presentation adult trauma will still come to Sunshine
All NOFs for operative management must be transferred to Footscray
Generally, if an adult presents to Sunshine who needs an operation, they will be transferred from Sunshine ED to Footscray. There are some exceptions to this and adult patients can be admitted at Sunshine if:
Theatre access at Sunshine/JK and no theatre access at Footscray for unreasonably long time (speak with periop manager about access issues and get approval)
Consultant on call requests for patient to be admitted at Sunshine
Sunshine has a large ICU
No paeds ICU in JK transferred to RCH
Major paediatric trauma and trauma calls <14yrs goes to RCH (gen surg will accept trauma calls 14yrs and over)
Ortho Wards
JK Children’s Ward
Level 6
Accessible only via lifts/stairs at JK main entrance
Two main ends to the ward (each with doctor’s office)
We use end that is on right side when entering ward
Left side is mainly for paeds medicine patients
Mainly double rooms
Have phlebotomy rounds but nurses also take bloods and insert cannulas and paeds med residents/registrars also help
Patient folders are in top cupboards next to entrance to doctors office
Forms are in bottom cupboards
NIC phone: 9055 3069
Paeds reg phone: 8345 0631
Sunshine
2H is main ortho ward during covid (while Williamstown was closed)
Accessible via lifts or stairs just down from theatre

Patient’s also end up on 1A (main surgical ward for plastics/gen surg)

Ward rounds
Handover room / office at blue cross in paeds outpatients.
Generally done by reg on call for that day and the reg the day before
Start time varies based on how many patients and where the registrars need to go after (e.g. paeds clinic vs Footscray clinic)
Registrars inform residents via whatsapp group day before for start time and location
Residents prepare google drive (separate Sunshine one) and audit list
Paper round first in JK meeting room
Weekends
Registrar is on their own
Occasionally you’ll have one of your sunshine residents/interns on for the weekend to help with ward round but they also cover all of surgical specialties so are very busy
Can speak with special surg cover via switchboard if needed
Theatre/DPU/DOSA
Only computers for templating are in JK – theatre 1 and 2 + anaesthetic bays
JK
Level 2 in JK (whole level is theatre)
Accessible via lifts/stairs next to JK main entrance OR via walkway from level 2 of Sunshine through the old paediatrics ward
Staffed 24/7
4 theatres + code green emergency caesar theatre
JK theatre NIC phone: 0435 197 524
Anaesthetics in charge phone: 9055 3128
Almost exclusively obs/gynae and ortho
Obs/gynae get preference for almost all cases and will often bump you with code green/pink (Caesar and obstetric emergencies)
Saturday/Sunday mornings are extremely hard to get into theatre as O&G do a meeting at 8:30 and theatre can’t start until they inform NIC + anaesthetics there are no pending caesars
As such, often better to just book cases to start at 12 when additional theatre staff come on and they can run two theatres + code green theatre
Loans gear is next to CSSD out back of theatre
Check theatre’s list order as this will often be different to booked list order
DOSA = day of surgery admission ?? JK’s DPU
Adjacent to theatre
Where you will find elective cases awaiting surgery that day
Allowed to do adult trauma at JK for emergency cases (e.g. septic arthritis/compartment syndrome that comes in via ED) or for pre-planned trauma cases approved by peri-op manager + bookings
Sunshine
Level 1 in Sunshine – north east side of hospital
Accessible via main lifts/stairs in centre of sunshine hospital next to main shop

5 theatres + endoscopy
Sunshine theatre NIC phone: 8345 1560
Anaesthetics in charge phone: 8345 0058/9055 3021
Mainly plastics/gen surg
We were doing some arthroplasty/elective stuff while Williamstown was closed but likely to dry up and be no longer used for ortho
Only used for ortho trauma when JK completely full and Sunshine has space for a case that needs to be done
Sunshine has a DPU attached to it – mainly used by plastics
ED
Only Sunshine has an ED
On south west side of hospital
Only accessible through radiology
Split into adults and paeds side
4 resus bays
Will do most reductions in resus or paeds treatment room if you ask
Not as proactive in doing adult reductions as Footscray
Generally under nitrous + IN fentanyl for paeds
