Clinic D

Purpose

All patients who are consented for surgery on Friday morning are sent by their doctor to Clinic D with their blue Consent Form (Request for Admission, RFA)
Clinic D Staff
- Check all paperwork
- Patients complete pre-operative questionnaires (PROMs – VAS pain, EQ-5D and joint-specific score).
- Enrol in research if relevant
- Provide educational resources eg. Orthoanswer

There is previously a roster for HMOs to ensure one is in Clinic D on Friday to fill out the campus suitability form. This has changed as of March 2025. Surgeons without a Footscray list will need to complete the Campus Suitability Asssessment form with the RFA

Complete the Campus Suitability Assessment Form for all patients who have consented for a joint replacement that day.
Additional cheat sheet (Campus Suitability Process help sheet) is attached to the front of the campus suitability form.
It lists the surgeons who only have a WTN list.
If the patient is found to be likely to require Footscray on the Campus Suitability form but their surgeon only operates at WTN, please find a footscray list surgeon as indicated by the SLN on a Friday morning via the Whatsapp Group (Ahead of the Curve)
