Root Cause Analysis
- Lack of Anaesthetic Room

Counter-Measures
Root Cause 19 – No sufficient anaesthetic rooms
- Trial of Anaesthetic Room (change storage rooms to anaesthetic room)
- Theatre Setup
- The computers table / seating to be removed from rooms 2, 3 & 4 & replaced with WOWs
- Provision/supply of 3 x Trolleys / 3 x Portable monitoring equipment / 3 x Cannula,
- Check the suction work (supply portables) & Emergency buzzer & rectify the issues, if present
- 3 x door upsize & replace to sliding doors instead, #2 Anaesthetic room door repositioning at the front, 3 x repositioning of the oxygen equipment, 3 x wall mounting of the phones
- Costs
- 3 x Trolleys (availability for initial trial/ purchase)/ 3 x Portable monitoring equipment / 3 x Cannula,
- Check the suction work (supply portables) & Emergency buzzer & rectify the issues, if present
- 3 x Provide portable computer trolley /WOWs (JB)
- 3 x door upsize & replace to sliding instead, #2 Anaesthetic room door repositioning, 3 x repositioning of the oxygen equipment, 3 x wall mounting of the phones
- Repositioning of the sinks (behind the current door)

KPIs
- On time start First Morning sessions (planned session time vs actual session start time)
- Surgery start (8:30am and also we will measure against 8:45)
- Anaesthesia start (8:15)
- 1st case: Clerical to nurse admission (<30min)
- 1st case: Nurse admission to anaesthesia start (<30min)
- Underruns: (No. of sessions finishing >30mins before 12:30 and 1730)
- Overruns: (No. of sessions finishing >30mins after 1230 and 1730)
