Root Cause Analysis

Counter-Measures

Root Cause 19 – No sufficient anaesthetic rooms

  • Trial of Anaesthetic Room (change storage rooms to anaesthetic room)
  • Theatre Setup
    • The computers table / seating to be removed from rooms 2, 3 & 4 & replaced with WOWs
    • Provision/supply of 3 x Trolleys / 3 x Portable monitoring equipment / 3 x Cannula,
    • Check the suction work (supply portables) & Emergency buzzer & rectify the issues, if present
    • 3 x door upsize & replace to sliding doors instead, #2 Anaesthetic room door repositioning at the front, 3 x repositioning of the oxygen equipment,  3 x wall mounting of the phones
  • Costs
    • 3 x Trolleys (availability for initial trial/ purchase)/ 3 x Portable monitoring equipment / 3 x Cannula,
    • Check the suction work (supply portables) & Emergency buzzer & rectify the issues, if present
    • 3 x Provide portable computer trolley /WOWs (JB)
    • 3 x door upsize & replace to sliding instead, #2 Anaesthetic room door repositioning, 3 x repositioning of the oxygen equipment,  3 x wall mounting of the phones
    • Repositioning of the sinks (behind the current door)

KPIs

  • On time start First Morning sessions (planned session time vs actual session start time)
  • Surgery start (8:30am and also we will measure against 8:45)
  • Anaesthesia start (8:15)
  • 1st case: Clerical to nurse admission (<30min)
  • 1st case: Nurse admission to anaesthesia start (<30min)
  • Underruns: (No. of sessions finishing >30mins before 12:30 and 1730)
  • Overruns: (No. of sessions finishing >30mins after 1230 and 1730)
